I, the undersigned, certify that I am the person identified by the driver's license presented and that the information provided on this form is true and accurate. I further certify that I am of legal driving age in the state of issue for my driver's license, and I am in good mental and physical condition to safely operate a motor vehicle. I understand that I am solely responsible for my actions and any damage or liability resulting from my operation of the vehicle during the test drive. I agree to abide by all traffic laws and regulations. I further agree to indemnify and hold harmless Affordable Dream Cars LLC from any claims, liabilities, costs, and expenses arising from my participation in the test drive. I will return the vehicle in the same condition I received it, and within the agreed time frame.
